Move Together
Move Together is a county wide initiative co-ordinated by Active Oxfordshire and delivered by district councils across Oxfordshire. The Move Together pathway is free to join and offers a person centred approach to support and motivate individuals to move more, whether it be at home, in the community, in the great outdoors or at a leisure facility.
The pathway provides personalised support for people who are inactive and/or have long term physical or mental health conditions, such as diabetes through phone calls, emails, online, face to face visits and coffee mornings to help people become more active.
A wide range of support and activity choice is available, including home activity packs, phone call support, community activities, exercise sessions for specific conditions, online sessions, digital apps, dance packs, exercise on referral options and much more. The pathway offers some free, low cost and discounted activity sessions in a range of places and environments to ensure choice for all.
Individuals can self-refer onto the pathway, by completing the brief enquiry form via the below link where one of our team will get in touch shortly.
Look out for upcoming drop-ins and activities for all ages at Unit 24. Move Together and Active Communities events are regularly updated in the window and on our Active Communities’ Facebook page.
Health professionals are also welcome to refer clients into the pathway, but we do encourage you to inform your client what you are referring them to and that it involves the team phoning or emailing them to have an initial conversation about what we can offer and how we can support.
If you would like to speak to one of the Active Communities team about how Move Together can help you, please email active.communities@southandvale.gov.uk or call Ashley Chapman on 07717714477.
You can find our classes and more information about Move Together on the Active Oxfordshire website.
